Openings Available in 3s Classes and Afternoon 4s PreK, Including UPK Spots

We still have openings in most of our classes. We are using an online enrollment form this year to make it easier for our families, but you will need to use a computer or laptop. (Unfortunately, tablets and phones won’t work with the website.)

The online form requires immediately emailing a photo of your child after form submission to [email protected] and paying the $75 non-refundable enrollment fee within 48 hours. 

First-quarter tuition is due for 2.5, 3s, and 4s classes. Children enrolled in UPK do not have to pay tuition. Pay through Zelle ([email protected]), check, or cash.

Medical forms will be due by your child’s first day of school. Please see below for links to medical forms and more information.

What You’ll Need

• Use a laptop or computer. The online enrollment form doesn’t work well on a phone or tablet.
• Verify your email before beginning to fill out our online enrollment form.
• Your driver’s license.
• Your automobile insurance.
• Your child’s doctor, full address, and phone number.
• Your child’s dentist, full address, and phone number.
• The address, city, state, zip code, and phone number of anyone who is authorized to pick up your child, or an emergency contact.
• Parent’s employer name and employer address, city, state, and zip code.
• Decide how you will pay the $75 non-refundable enrollment fee – Zelle, check, or cash. (Zelle payments are made to: [email protected].)

  9:00 a.m.–12:00 p.m.

Your child’s medical forms are due by their first day of school.

• General Health Appraisal Form – Requires a doctor’s signature. Can be substituted with your doctor’s own form.
• Certificate of Immunization Form – Provide a list of your child’s immunizations or this provide this form completed by your doctor’s office.
  • Non-Medical Exemption Form – If your child is missing any immunizations.
  • Medical Exemption Form – If your child is missing any immunizations.
  • The immunization exemption forms must be signed by your pediatrician or you must take an online learning module.
• Submit medical forms in one of these ways:
  • Mail medical forms to Community Preschool, P.O. Box 1652, Loveland, CO 80539.
  • Email medical forms to  Ms. Tammy at [email protected].
  • Drop off medical forms on your child’s first day of class.
